Papillary Carcinoma Surgery: Stage I. A few days ago, I mentioned that I was going through a very serious health situation. Now, following the evaluations conducted by the surgical oncologist, he suspected signs of malignancy in both my thyroid and my left axillary lymph node. Honestly, receiving news like this is never pleasant; however, there’s no choice but to face it with the greatest possible sense of responsibility and seek out the best treatment options for a cure—which was done as quickly as possible so as not to waste any time.

Para recordar un poco, el Doctor Juan Carlos Cedeño, cirujano oncólogo; me realizó punción en los nódulos tiroideos y en el ganglio axilar, para realizar las respectivas biopsias. En la tiroides resultó positivo para malignidad y en el ganglio axilar sospecha de malignidad. En tal sentido; debía hacerse una nueva biopsia del ganglio, por lo que tenían que sacarlo en su totalidad; sin embargo, se realizó la cirugía ambulatoria, pero como hubo mucho dolor extrajo solo muestras de tejido. Todo dependía de ello para proceder a la cirugía de tiroides y el procedimiento posterior del ganglio en caso de que resultara positivo.
El jueves 3 de septiembre se obtuvo el resultado de la biopsia del ganglio, por lo que el resultado fue el siguiente: Posible Enfermedad Linfoproliferativa, posible linfoma. Definitivamente había que extraer el ganglio, porque se recomienda hacer otra biopsia bajo el estudio de inmunohistoquimica; estudio que daría el resultado definitivo, pero este estudio se realiza en Caracas. Entonces, era preciso realizar las cirugías lo más rápido posible, planteando la misma para el pasado 9 de septiembre; como es lógico se debía tener el dinero y se necesitaban aproximadamente unos 5000$ para realizar ambos procedimientos.
To recap briefly, Dr. Juan Carlos Cedeño, a surgical oncologist, performed fine-needle aspirations on my thyroid nodules and axillary lymph node to obtain samples for biopsy. The thyroid sample tested positive for malignancy, and the axillary lymph node sample was suspected of being malignant. Consequently, a new biopsy of the lymph node was necessary, which meant it had to be removed entirely; however, although the procedure was performed on an outpatient basis, the surgeon only extracted tissue samples due to severe pain. The decision to proceed with thyroid surgery and the subsequent lymph node procedure—should the results come back positive—depended entirely on these findings.
On Thursday, September 3, the results of the lymph node biopsy came back, and the findings were as follows: Possible lymphoproliferative disease, possible lymphoma. The lymph node definitely had to be removed, because another biopsy is recommended as part of an immunohistochemical study—a test that would provide a definitive result, but this study is performed in Caracas. Therefore, it was necessary to schedule the surgeries as quickly as possible, with the procedure set for September 9; naturally, we needed to have the funds available, and approximately $5,000 was required to perform both procedures.
La cirugía debía hacerse para no perder más tiempo; el doctor nos informa que exactamente debíamos tener en primera instancia unos 4575$; pero solo se habían recolectado 2200$ y no había más remedio que buscarlos de otra manera; pedimos prestado 2375$, no se como lo vamos a pagar, pero habia que solucionar. Lo importante es que se logró realizar la cirugía como estaba prevista para el 9 de septiembre. Al salir de la misma; el doctor me informa que el procedimiento se extendió 2 horas más, porque dos nódulos estaban muy pegados a las cuerdas vocales y se debía actuar con mucha paciencia y delicadeza; por otra parte, el ganglio axilar estaba feo, necrótico, pero todo salió bien.
The surgery had to be done right away so we wouldn’t waste any more time; the doctor told us that we needed exactly $4,575 up front, but we had only raised $2,200, so we had no choice but to find the rest some other way; we borrowed $2,375—I don’t know how we’re going to pay it back, but we had to figure it out. The important thing is that the surgery was performed as scheduled on September 9. After the surgery, the doctor told me that the procedure took two extra hours because two nodules were very close to the vocal cords and he had to proceed with great patience and delicacy; furthermore, the axillary lymph node was in bad shape—necrotic—but everything turned out well.
Todo iba marchando bien y el día 10 de septiembre me dieron el alta; ya estoy en casa y se enviaron ambas muestras al laboratorio patológico para sus respectivos análisis. Se debe hacer la biopsia de la glándula tiroides completa, para determinar que variante es y así poder determinar el tratamiento preciso. Es decir; si solo se administra levotiroxina o si se debe suminitrar también el yodo radiactivo. Igualmente hay que determinar si lo del ganglio es linfoma; por lo que aquí harán una prueba preliminar para procesar las muestra, las devuelven con el informe y luego se deben enviar a Caracas para la prueba inmunohistoquimica.
Aquí dan el resultado el 14 de octubre y de inmediato se debe enviar para obtener el resultado final. Everything was going well, and on September 10, I was discharged; I’m now home, and both samples were sent to the pathology lab for their respective analyses. A biopsy of the entire thyroid gland must be performed to determine the specific type of tumor and thus decide on the appropriate treatment. In other words, whether to administer only levothyroxine or whether radioactive iodine should also be administered. We also need to determine whether the lymph node issue is lymphoma; so here they’ll perform a preliminary test to process the samples, return them with the report, and then send them to Caracas for immunohistochemical testing.
The results will be released here on October 14, and they must be submitted immediately to obtain the final result.
